Client Services

• Collaborate with Medicaid, hospitals/associations, provider entities and plans to develop and administratively manage standardized data exchange and interventional support services to reduce preventable events, improve quality measures and care transitions.
• Establish partnerships that facilitate integrated and coordinated care.
• Develop standardized operational models, and infrastructure components to ensure measurement of improved outcomes and utilization reductions.
• Design VBC models for providers and plans that create mutual incentives and ensure uniform measurement of improved outcomes and utilization reductions.
• Assist with development and implementation of revenue maximization and quality improvement strategies.
• Provide care management services (care coordination, comprehensive medication management) for providers and plans.
• Assist with identification of targeted patients and intervention models.
• Perform monthly reporting on defined metrics to track improvement in outcomes.
